Pre-op Procedure
If your pet is scheduled for surgery, it is important to follow these steps to ensure a safe, smooth, process:
Do not give the patient any food or water after 10:00 pm the night before surgery.
Plan to be present for a pre-surgical check-in with a technician.
Surgical admittance time is between 8:00 and 8:30 am.
Notify us of any medication given within 24 hours of surgery.
Provide your signature on surgical consent forms and contact information.
Expect a phone call from us after the procedure to set up a post-operative consultation appointment as well as patient pick-up time. We will review any specific at-home instructions with you during that time. In the event that your pet is staying with us for the night, we will provide you with regular updates during business hours.

I have read the forgoing and agree.
I understand that South Carolina state law requires rabies vaccination for all pets. I also understand that Shuler Vet Clinic and Mt. Pleasant Animal Hospital strongly recommend these additional vaccinations: DAPP, Bordetella (for canines), FVRCP and FELV (for felines). I understand that current Heartworm, Lyme, Ehrlichia and Anaplasmosis testing is required (for canines) and FELV, FIV and Heartworm testing (for felines).
I understand that all surgery/anesthesia involves some risk to my pet and that all precautions will be taken to minimize that risk. I agree to hold harmless, Shuler Vet Clinic, Mt. Pleasant Animal Hospital, and its agents for these risks.
I acknowledge that no guarantee or assurance has been made as to the potential outcome or results that may come from this anesthetic and surgical procedure.
I understand that I must be available via the phone number below throughout this entire day so that I can be reached in the event of an emergency or changes to the protocol.
